You can answer this question by plugging the given values into point-slope form, which is y - y_{1} = m (x - x_{1}). M will represent slope and x_{1} and y_{1} will represent the x and y coordinates.

   y - y_{1} = m (x - x_{1})   Substitute in the values
y - (-5) = 4 (m - 4)   Cancel out the -(-5)
   y + 5 = 4 (m - 4)   Use the Distributive Property
   y + 5 = 4m - 16    Subtract 5 from both sides
       
         y = 4m - 21
